Prerequisites
1.

You must have had no felony convictions within the three years prior to applying for this rotation.

2.

Non-U.S. Citizens must provide proof of Alien Residency or a work visa.

Rotation Preparation
Phase 1

Upon being matched to this rotation you will need to do the following:

1.

Complete the online Internship Enrollment Form.

2.

Attend a two-hour orientation class. You will be notified regarding the date and time once your name has been added to the roster.

Phase 2

Two months prior to your rotation you will need to do the following:

1.

Complete all the required application forms.

Phase 3

You will be contacted via Email by Employee Services Department with instructions on how to complete the online application.

1.

Complete e-Qip online application.

Phase 4

Two weeks prior to your rotation you will need to do the following:

1.

Review the Internship Overview.

2.

Review the Internship Guidelines.

3.

Complete the online Internship Agreement.
